If you live within our practice area, you are welcome to register with us. Our reception staff will be happy to guide you through the process. Eligibility can be quickly confirmed from your address. Photographic ID, and documents with your address, such as a recent tenancy agreement, bank statement, or utility bill can help with registration.
You will be asked to complete a registration form which includes a health questionnaire. This can provide helpful information whilst we wait for your medical records to arrive from your previous doctor. You can also complete the registration form at home and bring it to the practice. Once the registration process is completed, you will be able to make an appointment with our team.
