In order to evaluate the services we provide the Department of Heath surveys a small proportion of its patients each year. The survey used is independent of The Limes Medical Centre, and will be sent to some of our patients via post. It might look a little like the picture below.
We would be grateful if you could complete this survey as accurately as possible should you receive it. Help with translation is available from the Department of Health – information on how to get this will be in the letter you receive.
